Tellimed Chronic Disease Management

Tellimed delivers chronic disease management services to general practitioners and health funds. Over the past six years Tellimed has provided coordinated care to thousands of chronic patients, helping to reinforce their GP’s treatment, keep them out of hospital and improve their quality of life. Tellimed delivers a range of health care services such as chronic disease management, screening and prevention, hospital in the home, discharge planning and monitoring services. We are currently introducing our programs into the Western Super Clinic.

Western Day Surgery

The Western Day Surgery is a private day hospital in St Albans. Part of the Western Super Clinic, it is an accredited 12 bed unit which supports surgeons in conducting a variety of day surgery procedures for their patients. The day surgery is easily accessible to patients from either side of Melbourne. We are located directly opposite the Sunshine Hospital, close to the Western Ring Road’s Furlong Road exit ramp.

Westgroup Medical

Half-owned by the Tellimed Group, Westgroup Medical is a large after hours general practice with multiple GPs and supporting allied health. It is part of the Western Super Clinic and is located at 149 Furlong Road St Albans, directly opposite the Sunshine Hospital and close to the Western Ring Road’s Furlong Road exit ramp. Women’s Imaging Victoria

Women’s Imaging Victoria is a women’s ultrasound and imaging service due to open in the Western Super Clinic in April 2008. Services will be provided by ultrasonologist Dr Charles Siles.
